Make the Diagnosis: Post-Transplant Predicament

(MedPage Today) -- Case Findings: A 25-year-old man with acute myelogenous leukemia underwent a stem cell transplant. Ten days after the transplant he developed fever, became unresponsive, and demonstrated left gaze deviation. A head CT scan showed hydrocephalus and an external ventricular drain was placed. The CSF was sent for culture and Gram stain. The Gram stain showed abundant polymorphonuclear cells (PMNs) and the organism shown here. Culture grew a medium, greenish, slightly alpha hemolytic (described as green steel ball looking) colony at 24 hours. The organism grew on blood agar and chocolate agar, but not MacConkey agar. The second image shows a Gram stain that was performed from the cultured organism. Biochemically, the organism was catalase negative, LAP positive, PYR positive, oxidase negative, and positive for the Lancefield D antigen. What organism caused the infection?
